I found this ongoing issue where the 1st form of the Boss of the Tower Summit disappears after teleporting, especially near the entrance of the boss area. This persists in version 3.0.2. Some players found that keeping the boss away from the entrance worked to avoid this. I only had problems when the boss was in the down/left side of the area.
